Privacy Policy

All Motion Physiotherapy (ABN 92 693 615 351) ("we", "us") is committed to protecting your privacy. We handle personal information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and, because we provide a health service, the additional protections that apply to health information, including the Health Records and Information Privacy Act 2002 (NSW).

What we collect

• Contact details: name, phone, email, address and suburb.
• Health information: your health history, referrals, assessment findings, treatment notes, progress reports and related records needed to provide safe physiotherapy care.
• Funding information: NDIS plan details and plan management type, or claim details for WorkCover, CTP or DVA where relevant.
• Enquiry details you submit through our website form, including how you heard about us and your preferred contact method.

Why we collect it

We collect this information to respond to your enquiries, assess and treat you safely, plan and review your care, prepare reports you request (such as functional capacity assessments and NDIS progress reports), claim or invoice against your funding, and meet our legal and professional record-keeping obligations.

How we hold and protect it

Clinical records are stored in secure, access-controlled practice management software. Website enquiries are delivered to our practice email via our website host's form service and are not stored on the public website itself. We take reasonable steps to protect all personal information from misuse, interference, loss and unauthorised access.

Who we share it with

We only disclose personal information:

• to people involved in your care with your consent, such as your GP, specialists, support coordinators or plan managers;
• to funding bodies (NDIA, insurers, DVA) where required to fund your care;
• to service providers who support the practice (such as practice software and website form hosting), bound by their own privacy obligations; or
• where required or authorised by law.

We never sell personal information.

Access, correction and complaints

You may request access to, or correction of, the personal information we hold about you at any time by emailing info@allmotionphysiotherapy.com.au or calling 0450 758 795. If you have a privacy concern, contact us first and we will respond promptly. If you are not satisfied with our response, you can complain to the Office of the Australian Information Commissioner (oaic.gov.au, 1300 363 992).

Website

Our website does not use advertising trackers. Our hosting provider may collect standard technical logs (such as IP address and browser type) to operate and secure the service. Links to third-party platforms (such as Google or Instagram) are governed by those platforms' own privacy policies.
